5. Turn in Health Clearances
The Department of Health requires that all students provide health clearances (tuberculosis and MMR) before they are permitted to register for classes (Hawai‘i Administrative Rules, DOH Title 11, Chapter 157). To prevent unauthorized registration, a health clearance hold is placed on all student accounts and cleared once proper documentation is submitted.
You may elect to bring in your own shot records or use this form: Health Clearance Form
Tuberculosis (TB) – All students must provide evidence that they are free of active TB. The clearance must be dated within one year of the first day of the semester and state that the skin test (PPD-Mantoux) or chest x-ray was negative. Transfer or returning students who are/were enrolled at a Hawai?i college may bring in a copy of the original clearance certificate used to first attend a post-secondary school in Hawai?i.
Measles, Mumps, Rubella (MMR) – All new, returning and transfer students must provide evidence of MMR immunization. Students born before 1957 are exempt. The clearance must indicate one of the following:
- Proof of two doses of measles (rubeola) vaccine; at least one must be a measles, mumps, rubella (MMR) vaccine. The first dose must have been given on or after your first birthday
- Positive IgG blood test report proving immunity of the blood antibody titers against MMR
- Documented history of the disease
